General Explanations

1. Data subjects defined as “relevant persons” under the Personal Data Protection Law No. 6698 (“the Law”) (“Applicant”) are granted, under Article 11 of the Law, the right to submit certain requests to data controllers regarding the processing of their personal data.

2. In accordance with the first paragraph of Article 13 of the Law, applications made to Pixwrap, as the data controller, regarding these rights must be submitted in writing.

3. In this context, written applications to Pixwrap may be delivered by printing out this form, completing the sections indicated below and signing the pages, through:

• Personal application by the Applicant,

• Through a notary public,

• By signing with a secure electronic signature, as defined in the Electronic Signature Law No. 5070, belonging to the Applicant, and sending it to the Company’s registered electronic mail (KEP) address.

4. Applications made in person with a wet-ink signature or through a notary public must be sent to the “Address” address. In both forms of application, the envelope sent to Pixwrap must be marked with the note “Information request under the Personal Data Protection Law.” Applications made via secure electronic signature must be sent to Pixwrap’s KEP address (abat@hs01.kep.tr), with the subject line of the e-mail marked “Information request under the Personal Data Protection Law.”

5. If the Personal Data Protection Board (“the Board”) determines and announces any additional application methods, announcements regarding these methods will be made via Pixwrap’s website.

6. In accordance with paragraph 2 of Article 13 of the Law, your applications submitted to Pixwrap will be answered, depending on the nature of the request, within 30 (thirty) days from the date your request reaches Pixwrap. Pixwrap’s responses will be delivered to you in writing or electronically, again in accordance with Article 13 of the Law.

7. If your application requires an additional cost, you will be required to pay the fee determined by the Communiqué on the Procedures and Principles of Application to the Data Controller issued by the Personal Data Protection Board. If a written response is given to your application, no fee will be charged for the first 10 (ten) pages; a processing fee of 1 TL will be charged for each page beyond 10 (ten) pages. If the response to your application is provided on a recording medium such as a CD or flash drive, the cost of the recording medium will be charged to you.

8. Pixwrap reserves the right to make changes in line with Board decisions regarding application procedures or future legal regulations.

This form has been prepared in order to determine your relationship with Pixwrap, to fully identify any personal data processed by Pixwrap, if any, and to be able to respond to your application correctly and within the legal timeframe. In order to eliminate legal risks that may arise from unlawful and unfair data sharing, and in particular to ensure the security of your personal data, Pixwrap reserves the right to request additional documents and information (such as a copy of your ID card, passport or driving license) for identity and authority verification. If the information you provide under this form is inaccurate or outdated, or if the application is made by an unauthorized person, Pixwrap accepts no liability for claims arising from such incorrect information or unauthorized application.
